Program data shared in this form will be reflected on the FCSS Community Impact tool. To see your impact directly, visit the Alberta View page and toggle filters to see your community. A comprehensive view of data from across Alberta is presented on the main page and each of the Provincial Prevention Priorities pages. By inputting data, you’re certifying all information provided about your program(s) is correct and accurate to the best of your knowledge. This form will automatically populate a date stamp upon submission to the database.
